Think it will be totaled because of hitch?

No. A bent hitch does not equal a total.

    What year is it?

I've had worse than that repaired to like-new condition. Good luck!

Ackshually... If that rear tube the hitch is bolted to gets bent, they will typically total them because it means it needs a frame. No it shouldnt, yes it should be able to be fixed, but since the rear cross tube is welded to the frame rails, they pretty much total them because they are not allowed to cut them out and weld another one in, or straighten them.
You would have to have a really new vehicle with a high MSRP / black book value to not get totaled in a rear end collision. You can look on COPART at all the newer 4runners totaled with 30,000 miles from a rear end. Most of them look so puny like nothing is wrong, dont matter totaled.
I hope you end up on the good end of things pal but.... from the historical data i have seen over the years, things dont look good for you.

Ackchyually, I think the rear frame can be sectioned. I think it's an accepted repair to replace the rear crossmember/hitch.

I’m guessing it’s the fault of the other driver?

dang, news to me, for some reason it surely isnt done. I have seen dozens and dozens of 5th gen 4R with awfully low mileage that were rear ended at COPART with salvage titles going up for auction. Most of these look freakin great and need a rear door skin and bumper cover and they will be ready for a few hundred thousand miles more. Seen way too many like this:

2023 TRD Sport with 15,300 miles totaled from a rear end

And this 2023 TRD Sport with 12,300 miles totaled from a rear end, it looks perfect actually.
    https://www.copart.com/lot/51115414/salvage-2023-toyota-4runner-trd-sport-tx-austin

and this 2023 TRD Sport with 13,800 miles totaled from a rear end, it looks so good i wanna go get it and put a bumper on it and keep riding.

ouch... this 2024 Limited with redwood interior, 4wd, 3rd row, with 3,300 miles that needs about $3500 in body and paint
    https://www.copart.com/lot/55659324/salvage-2023-toyota-4runner-se-ct-hartford

how about a 2021 SR5 premium that needs a rear quarter panel and a bumper cover. $2500 in repairs... maybe another tire this one is flat.

this one is weird, a 2023 Limited 4wd 3rd row redwood interior totaled from vandalism? a few windows? No torn seats? weird..

At least a couple of those "quarter panel" repairs look like roof damage to me (they are probably buckled up top). If the quarters need to be replaced, it's a lot more than $2500 or $3500 to repair, and you'll have time into the roof. The shop is going to want to paint over the roof, claiming the quarters won't match, and if the quarters are damaged, the paint has to go into the adjacent door. The gaps are all off, which means frame time, in addition to the frame section. I think the OP is good, unless the salvage value comes back too high. Also the OP didn't fill out their location in their profile, and as insurance is state-specific, it's really anyone's guess.

I like how the rear end looks without all that extra plastic.

Dude... Where are you from? No body shop blends from a rear quarter into the adjacent door anymore, NOBODY. A rear quarter is going to get blended about 4" up the C and D pillar AT BEST. Drive around and look at cars ALL OVER with different color fenders and doors, rear bumpers that dont match the trunk lid. Insurance companies are doing rock bottom pricing on fixes and NO body shop can afford the time and labor from doing the corrections you are talking about. There is not a body shop in this country that is going to blend a lower rear quarter up to the roof, zero chance of it happening. Do you want that type of quality on your vehicle, sure, but are you getting it, absolutely not.

You must be a really good painter, and I don't know how you clearcoat that, but ok. You do a lot better with 4" than I could. I don't know of a decent shop that will provide a warranty on the repair you're talking about, but if you can do that kind of work, I hope you're making a lot of money doing it. You deserve it.

Not blending from a quarter replacement into the adjacent door would take more time and material color-matching, with let-down panels, than just blending, and I've never seen a quarter replacement that wasn't bare metal within 4" of the weld. I don't know if you're a painter or body tech, maybe both, but you're not just good, you're the best I've ever heard about.
