I guess generally speaking but mostly would it help in bottom end and mid range torque? I occasionally tow a 4500lb travel trailer. It does ok about what I would expect but can't help but wonder if it could be better? (without major engine modifications)

best thing would be to regear

Kyle is right, your best results will come from a re-gear as the tune just isn't going to change much for your intended purpose.

I have a travel trailer that has a GVWR of about the same as you. I also have the OTT tune. It helps slightly but I would not recommend the tune if you are only wanting it to help with towing as it is not worth it for that. My reasoning for the tune was to improve the gear hunting while using cruise control on the highway and the throttle response of the 4R, which it has.(Not while towing though) While we can tow around the 4500lb mark, it is not pleasant and hills are a struggle. If it is doable, get a bigger tow vehicle.
