Missing when going 40-65mph -

    1985 Toyota 4Runner
    rebuilt from ground up - including engine -
    Any ideas why the engine seems to miss once I reach 40-45mph - it seems to stop if I go faster - and if I put into OD - also seems to do it when engine cold -
