Horn only works turning

2013 Limited horn works when turning >20° to right or left. Driving straight horn doesn't work. Suspect defective clock spring. Would a bad clock spring still allow operation when turning? Based on other forums I was wondering if this could be a ground issue. 4Runner has ~90,000 miles.
