There was a post on the topic of this dilemma but specifically pertaining to 17" wheels.

But wanted some help on tire size with 18" wheels.

The shop was going to place an order on my Fuel Vapor 18 (20 offset) with KO2 265/265/18 but was just informed a few mins ago those tires are on back-order and it was recommended to get the 275/65/18 instead. I feel like I'm just going to agonize over this now after I made up my mind lol.

+ Should I consider the 275 instead? I have no intention to raise my 4Runner until a couple of years down the road. Its just a 2018 SR5 stock.

+ The Vapors are heavy wheels, so now I'm adding another 3-4lbs each by simply moving up 265 to 275. Prob no big deal?

+ The Vapors are also 9" wide wheels

Will I notice any difference in ride? gas mileage? I don't do any rock crawling.

Last I checked they only offered the 275 size in load range E, which will make a big difference in ride/mpg if you weren't already looking at that. I don't have the bandwidth right now to look up the specs on tiresize.com, but I would look there. I wouldn't worry about the effect of the circumfrance difference, your talking a pretty small jump.
