    So,

The short story is simple. If I ever, ever install rear shock mounts again I'm going to cut a hole in the body and install a rubber grommet for removal to get to the top nut on a shock mount in a 3rd gen.

Lets see what you all have come up with to deal with the silly shock design.

Is this design issue specific to only a few of the 4R generations?

You could do a body lift. Not sure how big you need to do to alleviate this but I am planning 1.5" eventually.

This is part of the reason why I have a BL. Now the one I worked on did not. If there is a 2in BL it makes it easy but I"m just cutting a hole next time and going to be done with it.

You could always outboard your shocks on the outside of the frame. Leave the lower mounting point as-is and bypass the factory upper mount altogether.
