I purchased a 100,000 mile extended warranty at the dealership when I bought my 2018 4Runner TRD Off Road new. I've got 50,000 miles on the vehicle now. Warranty terms require me to perform all maintenance as required "by the manufacturer." That would be Toyota. Warranty is underwritten by a reputable insurance company. However, service records are required upon filing a claim. How nitpicking are they liable to go in order to deny a claim? Any experiences? The dealership is loading up on "recommended" maintenance but that is them and not what is required by Toyota, which I have done. Did I waste my money? I can still get half of the premium back. (Be gentle.)

Depends on how much you paid. Any deductibles. Sometimes these third parties would force you to pay for all repairs and start chasing them with invoice claims.

Personally i would only buy factory extended warranties.
